5 surprising reasons why you're not sleeping through the night

Most explanations for poor sleep point to stress, screens, and caffeine. These are real factors, but they are rarely the actual reason why a woman falls asleep easily and then wakes up at 3 AM for no apparent reason. Here are five reasons that explain exactly that, and why almost none of them appear in common advice.

1. Your internal clock is losing precision, not your willpower

Sleep is regulated by the circadian clock, an internal pacemaker that determines when the body enters deep sleep, when it goes through REM phases, and when it should sleep through the night. This clock runs on a protein called SIRT1, and SIRT1 cannot function without sufficient amounts of a coenzyme called NAD+. NAD+ continuously declines in every adult from their mid-20s, and by middle age, most people operate at about half of what they had at 20. As NAD+ decreases, SIRT1 activity declines with it, and the clock that is supposed to keep your sleep together loses accuracy. This is a documented mechanism of aging research, not a theory, and one of the most overlooked explanations for why sleep deteriorates over the years without any apparent external cause.

2. Your hormones are more directly linked to sleep than most people think

Progesterone has a genuine sleep-promoting effect, and its production relies on an enzyme that requires NAD+ as a cofactor. The hormone most responsible for restful sleep in the second half of the cycle is therefore not just a hormonal issue, but also a cellular one. If NAD+ availability decreases, progesterone synthesis becomes less efficient. This is part of the explanation why sleep in the luteal phase and later in perimenopause tends to become more difficult with age, even if nothing else in your routine has changed.

3. Your body never properly cools down

Core body temperature must drop by about one to two degrees for the body to initiate and maintain deep sleep. Overly warm bedrooms, heavy duvets, and even a hot shower shortly before bed can delay or prevent this drop. This manifests as difficulty falling into deep sleep or waking up at night while the body again tries to reach the temperature threshold it missed the first time. This is one of the simplest and most easily rectifiable mechanisms on this list, and one of the least discussed.

4. Blood sugar fluctuations wake you up

A drop in blood sugar during the night triggers the release of cortisol and adrenaline, the body's standard reaction to a perceived energy deficit. Both hormones are incompatible with deep, continuous sleep. An eating pattern that causes a sharp drop in blood sugar at night, often due to a carbohydrate-heavy meal shortly before bed or insufficient food throughout the day, can produce an awakening that feels exactly like stress or anxiety, but is in reality metabolic.

5. You don't breathe as freely at night as you think

Partial narrowing of the airways during sleep is significantly more common than diagnosed sleep apnea and is rarely investigated because it doesn't always produce loud snoring or an obvious daytime warning sign. Shallow, restricted nasal breathing reduces the amount of oxygen reaching the blood at night. This can be enough to trigger short, unremembered arousal reactions that fragment sleep without you ever fully waking up. The result is a night that appears complete on the surface but doesn't provide the depth of rest it should. Some women find that simply facilitating nasal breathing overnight, for example with simple nasal strips, makes a noticeable difference in how complete their sleep feels. However, this is a mechanical aid for airflow and not a treatment for a diagnosed condition. Anyone who suspects sleep apnea should seek medical clarification.

The pattern behind all five

None of these five causes is a matter of discipline, and none can be solved by trying harder to relax. They are biological mechanisms, some hormonal, some metabolic, some mechanical. Understanding which of these is actually disrupting your specific sleep pattern is the difference between years of guessing and targeted remediation of the actual cause.
